INFORMED CONSENT TO PARTICIPATE

You have chosen to participate in Baby 101, an educational class designed to address parents’ most common concerns about 1-month-old babies. This class is taught via a video call by the professionals at Eugene Pediatric Associates (EPA) in Eugene, Oregon.

Participants will need to rename themselves on the video call to only be the first name. Any information about you that is shared with the group will only be what you willingly share. Any information shared will not be shared with anyone outside the class. Please be advised that although EPA will take every precaution to maintain confidentially, the nature of group education prevents EPA from guaranteeing confidentiality.

Your participation is entirely voluntary and you are free to choose not to participate.
